Prevention measures eldery people (60-65 and 75+) in Stadskanaal

This demonstration project is implementing activities within the theme “service provision – do more with ICT” and with the theme “innovative housing and provision of care services”. 
 

Short description of the pilot

 

This project is based on the premise that by preventative interventions Stadskanaals’ senior citizens should be able to live at home, independently, for a longer period of time. When building new homes and neighbourhoods, it is possible to take the limitations of elderly people into account. With existing houses there is still a challenge to make it possible and affordable for elderly people to live in their own familiar surroundings for as long as they wish to do so. The municipality, together with Stichting Welstad will collect a large amount of information about the circumstances under which interventions in the living conditions are made. An important side-effect will be the growing awareness within the target group. The project is set against the background of an ageing and decreasing population, in an area that is for the most part urbanized (and that has all the according facilities). About 30% of the municipality Stadskanaal is rural and sparsely populated, with limited facilities. 
 

Main aim

 

The main aim is to inform and advise elderly people about measures that may help to improve their health. At the same time the (preventative) measures will be taken to improve the health of people from the target group, or to at least to prevent a worsening of their situation. 
 

Expected results

 

First of all, an increased self reliance and hence less need for more serious forms of service and health care is expected. Furthermore, a set of measures aimed at preventing fall incidents and other avoidable health issues and significant improvement of the general well being and sense of security of the elderly. Another result will be an adapted questionnaire (as used by senior advisors), more aimed at prevention and a system with the necessary software to closely monitor and coordinate different signals in the field of health care and general well being.
